Please note if you are already credentialed in life care planning, you do not need to submit a redacted life care plan. However please assist us by writing the examination. An application is considered to be incomplete until all information has been received & the fee paid. A current Certificate of Errors and Omissions/Liability Insurance must be received to finalize application for a credential. Due to COVID-19 CVRP is accepting soft copies of VSPC/PC but reserves the right to request a hard copy.

The grade achieved from the examination does not affect your ability to achieve the ICCCP(F) credential. The examination is currently in beta for peer review to collect a substantial sample size. A successful ICCCP(F) applicant will be awarded 12 ICCCP CEUs for participation in life care planning examination in beta version. The credential will be awarded to the applicant once the examination is completed.

In addition 3 of the following 6 categories must be met.

E) Please attach proof that you taught life care planning certification-related courses at or through a recognized institution; &/or

F) Please attach proof that you made presentations as a subject matter expert at recognized conferences where a minimum of 8 certification education maintenance credit hours in total were awarded (including credit for speaker preparation hours) or would be eligible for awarding of CEUs; &/or

G) Please attach proof that you have experience in doing peer-reviewing for life care planning credentialing; &/or

The candidate is eligible for the ICCCP Fellowship, if candidate meets 3 of the 6 criteria for Fellowship as detailed in items “c” through “h” above.
